Nurse-led care models have emerged as an innovative and effective response to the challenges facing current healthcare systems, such as an aging population, increasing chronic diseases, and inequalities in access to care. These models, ranging from advanced practice to case management and specialty clinics, demonstrate the crucial role of nursing in providing comprehensive, quality care. The objective of this extensive study is to analyze successful experiences with nurse-led care models, identify evidence of their effectiveness, and extract lessons learned for their implementation and expansion. A systematic review of the literature, including quantitative and qualitative studies, was conducted to synthesize key findings. The results highlight improved access to care, patient satisfaction, chronic disease management, cost reduction, and optimized care coordination. The lessons learned emphasize the importance of leadership, interdisciplinary collaboration, advanced training, nurse empowerment, and the creation of supportive practice environments. In conclusion, nurse-led care models represent a promising strategy for strengthening health systems and improving patient outcomes, with significant implications for nursing practice, policy, and research.
